Insurance
We are committed to providing you with the best possible care. If you have medical insurance, we wish to help you receive your maximum allowable benefits. To achieve this, we need your understanding of and assistance with our financial and payment policy.
We will happily bill your insurance company (including private insurance, Tricare, Medicaid and Medicare) but will need you to provide us with your active insurance card at the time of your appointment. Payment is expected at the time of service. This is an insurance company rule and includes co-payments or coinsurances. For your convenience we accept cash, check, or credit card (Visa or MasterCard).
If you do not have insurance or wish to pay out of pocket at the time of your appointment please call our office to discuss discounts and payment plans.
We will provide a Good Faith Estimate to all patients, insured or non-insured, at the patient's request. Please request your estimate at least 3 days in advance of your appointment date. If you're unsure of your insurance coverage for specific services, please review our fee schedule here and contact your insurance company with the 5-digit CPT code to determine your out-of-pocket responsibility.
Accepted Types of Payment
We accept cash, check, VISA and MasterCard credit cards. Payment in full at the time of service is required in the following circumstances:
- You are a self pay patient (payment arrangements can be set up if you are not able to pay in full at time of service).
- You have not brought your insurance card(s) with you.
- You have not met your deductible.
To avoid a cancellation fee, please cancel at least 2 business days in advance of your first visit or follow-up appointment and at least 5 business days in advance of your testing/procedure appointment.
