We are happy to assist with billing your insurance and want to ensure you are familiar with our financial policies. Here's what you need to know: 

Insurance Billing

We are committed to helping you receive your maximum allowable benefits. We will gladly bill your insurance company, including private insurance, Tricare, Medicaid, and Medicare. Please remember to bring your active insurance card to your appointment. Payment is expected at the time of service, including balances, unmet deductibles, co-payments, coinsurances, and noncovered services. For your convenience, we accept cash, check, or credit card (Visa or MasterCard).

Self-Payment and Discounts

If you do not have insurance or prefer to pay out of pocket, please contact our office before your appointment to discuss discounts and financial assistance that may be available to you.

Fee Schedule and Estimates

At your request, we will provide a Good Faith Estimate to all patients, insured or uninsured, at least three days before your appointment. If you are unsure about your insurance coverage for specific services, please review our fee schedule and contact your insurance company with the 5-digit CPT code to determine your out-of-pocket responsibility.

Accepted Types of Payment

We accept cash, checks, and VISA and MasterCard credit cards. Payment in full at the time of service is required in the following circumstances:

  • You are a self-pay patient.
  • You have not brought your insurance card(s) with you.
  • You have not met your deductible.
  • You have copay or coinsurance obligations.

Appointment Cancellations

We kindly request a minimum of 2 business days' notice for any appointment cancellations or rescheduling requests. Failure to provide this notice will result in a $50 rebooking fee that must be paid by the patient before rescheduling. Please note this fee will not be billed to insurance and is solely payable by the patient.
